Every shift requires hundreds of safety-critical decisions across permits, isolations, alarms and handovers. Yet how many are made with complete, up-to-date information?

When the U.S. Chemical Safety Board investigates a major incident, it rarely identifies an unknowable hazard. More often, investigators find information that already existed within the facility: an earlier alarm, a bypass unknown to the permit writer or an outdated procedure that no longer reflected the equipment. The information simply failed to reach the right person at the right time. An Octave analysis of 35 CSB-investigated incidents found that more than 90% involved at least one information disconnect, while none resulted from a truly unknowable hazard.

The 2019 PES Philadelphia refinery fire, which caused roughly $750 million in losses, involved piping that had not been re-evaluated after the governing standard changed.

Join us for an exclusive webinar session exploring familiar scenarios involving combined risks, the broader patterns behind them and an operational approach to closing critical information gaps. Using a connect, contextualize, act and prove framework, teams can better understand operating conditions, deliver timely information, preserve critical context during handovers and translate identified risks into documented actions.

Attendees will gain a practical framework, guidance on supporting industry standards and a clear view of capabilities available today and those still in development for safer, consistent decision-making.
